Responsibilities
- Provide comprehensive and personalized patient care.
- Perform prescribed care within the scope of practice.
- Participate in evaluation, monitoring and adaptation of the care plan.
- Contribute to coordination among professionals.
- Support patients and their families throughout the rehabilitation pathway.
- Participate in quality initiatives and practice improvement.
